In January 2026, Egypt Assistance managed a complex, rapidly evolving case involving a 69-year-old female insured patient in Sharm El Sheikh, Egypt.

The case began with non-specific symptoms — weakness, abdominal pain, fever and jaundice — initially assessed as an acute abdomen. Within 24 hours, the patient’s condition deteriorated significantly, prompting emergency hospitalisation and ICU admission with suspected shock, anaemia and obstructive jaundice.

As the clinical picture evolved, Egypt Assistance took an active coordination role across multiple levels. The patient was transferred from a private hospital to a state facility to ensure access to broader diagnostic capabilities. Continuous communication was maintained with treating physicians, while clinical decisions were aligned in real time with the insurer.

As the patient’s condition continued to deteriorate, the therapeutic window for ERCP became increasingly limited. Following multidisciplinary assessment, an alternative biliary drainage procedure was performed, resulting in temporary clinical stabilisation.

The case was further complicated by respiratory failure requiring mechanical ventilation, haemodynamic instability, gastrointestinal bleeding and suspected underlying malignancy.

Alongside the medical complexity, significant financial and logistical challenges emerged, as the patient’s relatives faced restrictions on international fund transfers. Egypt Assistance maintained close coordination with all stakeholders, including the insurance company, medical providers, the consulate and the family, to identify and implement viable payment solutions.

Despite all efforts, the patient’s condition remained critical, and she passed away on 22 January due to cardiac arrest.

Following the patient’s death, Egypt Assistance remained in close contact with the treating hospital to complete all outstanding administrative and operational requirements necessary for repatriation. Through this coordinated multi-stakeholder effort, hospital costs were successfully reduced to approximately USD 14,000, and payment was ultimately arranged via the insurer’s regional office, allowing the case to progress.

Key learning points:

This case highlights the importance of a multi-stakeholder assistance model in complex international claims. Effective case management required simultaneous coordination between medical providers, the insurer, the consulate and the patient’s relatives. Acting as a central coordination hub, the assistance company ensured alignment across all parties — balancing clinical realities with financial and logistical constraints to maintain continuity of care under highly challenging circumstances.

Egypt Assistance managed a time-sensitive cardiac emergency involving a 70-year-old male insured from Bulgaria, who was staying in Hammamet, Tunisia.

The insured presented with acute chest pain while at the hotel and contacted Egypt Assistance. Following an initial assessment by the hotel doctor, he was transferred by ambulance to Polyclinique Hammamet for further evaluation and hospital admission.

The insurance policy was quickly verified and confirmed as active. Based on the clinical presentation suggestive of an acute coronary syndrome, urgent authorization was granted for coronary angiography and a Guarantee of Payment (GOP) was issued without delay.

Coronary angiography revealed a significant stenosis in the distal right coronary artery. The cardiology team performed immediate percutaneous coronary intervention and successfully implanted an active stent in the affected vessel. The procedure was completed without complications, with good angiographic results.

The patient remained clinically stable and was discharged in good condition. A Fit-to-Fly certificate was issued, confirming medical clearance for air travel.

The patient was subsequently cleared for return travel on a commercial flight. However, due to the hospitalization period and missed original departure, Egypt Assistance arranged a new return flight in line with medical recommendations. A replacement ticket was organized to ensure a safe and smooth repatriation.

The insured returned home safely without further complications.

Operational Insight

This case highlights the importance of fast GOP approval in acute cardiac cases and the value of close coordination between medical providers, insurers, and assistance teams. Egypt Assistance ensured early intervention, structured discharge planning, and seamless repatriation coordination, enabling a safe clinical outcome and efficient cross-border transfer within a short timeframe.

The swift and coordinated response of the Egypt Assistance team ensured that all passengers involved in the bus accident received timely and appropriate care, highlighting the importance of efficient emergency management in remote and complex situations.

In November 2024, a severe bus accident occurred on the highway connecting Sharm El Sheikh and Cairo, near the cities of Ras Sedr and Suez, approximately 300 kilometers from Sharm El Sheikh. The crash, which involved 35 passengers – including both adults and children – resulted in multiple injuries ranging from minor to life-threatening. Among the most severe injuries was a traumatic amputation of an 11-year-old child's right arm.

The passengers, who were travelling for leisure, were caught in a horrific crash along a remote desert road, where medical resources were limited. The cause of the crash remains under investigation, but the severity of the incident necessitated a rapid and coordinated response to provide medical care to the injured.

Initial Medical Response and Coordination

Upon receiving notification of the accident, the Egypt Assistance operations safety and risk management team immediately dispatched a medical response unit to the site. Due to the remote location of the crash, which was over 100 kilometers from the nearest medical facility, immediate coordination with nearby hospitals in Ras Sedr, Suez, and Sharm El Sheikh was essential.

A specialized medical team arrived on site to assess the situation, triage the injured, and stabilize their condition. The most critical cases, including multiple fractures and post-concussion injuries, required urgent medical intervention. Medical evacuation was initiated immediately to transport those with severe injuries to hospitals capable of handling such cases. For patients with less critical injuries, transportation was arranged to ensure they received prompt care at the nearest appropriate medical facilities.

Medical Complications and Challenges

Among the most challenging cases was the traumatic amputation of an 11-year-old child's right arm. Despite swift medical evacuation to Suez Medical Complex, the limb could not be saved due to extensive injury and the lack of revascularization options. The child received stabilization and was successfully repatriated for continued care.

The proximity to hospitals in Ras Sedr, Suez, and Sharm El Sheikh, as well as the need for further treatment for several passengers, meant continued coordination with hospitals, insurance providers, and medical teams. This ensured smooth transitions for all patients, whether by ground or air ambulance, depending on their conditions.

Tourist and Travel Support for Families

In addition to the immediate medical response, Egypt Assistance demonstrated its capacity to provide comprehensive tourist and travel services. The team arranged two minibuses to transport patients and their families back to Sharm El Sheikh. Furthermore, the families were provided with hotel accommodation, local transportation, and air tickets for those needing to return to their home destinations, ensuring seamless travel arrangements during this challenging time.

The bus accident on the Sharm El Sheikh to Cairo highway presented considerable challenges due to its remote location and the severity of injuries sustained. However, the rapid and efficient emergency response, led by the Egypt Assistance team, ensured that all passengers received appropriate medical care. Through effective coordination, quick thinking, and the dedication of medical teams, all injured individuals were safely repatriated or transferred to specialized facilities for further treatment.

This case highlights the importance of preparedness and timely action in handling complex emergency situations in remote areas. The Egypt Assistance team's quick coordination and ability to mobilize resources, both medical and logistical, played a pivotal role in ensuring the safety and wellbeing of the affected individuals and their families.

In October 2023, at around 10 pm, an accident occurred involving a tourist bus carrying citizens from southeast Asia. We received the news from their insurance company and quickly initiated contact with the relevant authorities to gather information. We coordinated with the police in St Catherine, and St Catherine Hospital, to ascertain the facts of the accident, injuries, and potential fatalities. Unfortunately, one foreign national unrelated to our company was among the deceased.

Our next step was reaching out to the tourism company responsible for the group to obtain a preliminary list of tourists, including their names, nationalities, and dates of birth, if available. Subsequently, we maintained regular communication with the hospital through our medical assistance team to inquire about the status of the injured and obtain medical reports. Due to the severity of the accident and the high number of casualties, we expedited the issuance of medical reports by collaborating with our medical staff. We also liaised directly with emergency doctors at the hospital to determine treatment possibilities and the need for transfers to specialized facilities.

Transferring patients

Following extensive communication between our medical staff and our international assistance company, a decision was made to transfer all 26 tourists. Four were discharged after receiving necessary medical attention, while the remaining 22 were transported to Sharm El-Sheikh International Hospital, equipped with essential medical facilities like X-rays, CT scans, MRI machines, lab services, and specialized medical care. We also coordinated with ambulance services to ensure that each patient received the appropriate level of care during transport.

Through collaboration with the Ministry of Health and the hospital administration, the patients were efficiently received at Sharm El-Sheikh Hospital. A considerable number of emergency and specialized doctors were made available to address each patient’s needs based on their condition and priority. After approximately four hours at Sharm El-Sheikh Hospital, 12 patients were deemed stable and were discharged. Suitable transportation was arranged to take them back to their hotel in Sharm El-Sheikh.

Simultaneously, we contacted the police to recover the patients’ belongings and directed them to the respective hotels where the injured tourists and their families were staying. The medical team at St Catherine Hospital determined that 10 patients needed to be transferred to Cairo for surgeries not available in Sharm El-Sheikh. Two patients were transported via air ambulance, while the rest were accommodated in ambulances and a minibus for their families and transferred to AS-Salam International Hospital in Cairo.

The highest level of care

We ensured the highest level of care during transportation, with ALS ambulances and medical personnel accompanying the patients. Proper permits were obtained for both patients and medical staff for smooth entry into the airport. After flying to Cairo, they arrived at the hospital in the evening.

Our team secured accommodation for an overnight stay and directed the relatives of some patients to the nearest hotel for easy access to their loved ones. Ground ambulances continued to arrive at the hospital, with each case assessed in the emergency department to determine the necessary interventions, including surgical procedures.

Medical permits were obtained in Sharm El-Sheikh to facilitate the injured travelers’ departure from the airport the following day. In Cairo, our medical team maintained continuous contact with the hospital staff to monitor patient progress.

A day later, seven patients were discharged to their relatives’ hotel. Two days after that, one more patient was discharged. Four patients remained in the hospital for an additional seven days, after which the consulate contacted us to arrange the return of all injured tourists and their families to their home country.

Many expressions of gratitude were received for managing such a crisis, from the initial accident to the safe return of injured tourists to their home countries. Thanks to the dedicated efforts and support of our global and local network, we were able to deliver high-quality assistance to international patients and their family members.

The vehicle had significant front-end damage, presenting a high risk of further deterioration during handling and transport. The situation required precise coordination, expert handling, and a fully equipped tow truck to guarantee complete safety.

Our Action:

Our Customer Service team acted immediately, accurately documenting all critical details, including vehicle condition, exact location, and final destination. Within record time, a fully equipped tow truck arrived on-site. Using advanced fixation equipment, the vehicle was securely stabilized to prevent any movement or additional damage during loading. Despite the severity of the accident, the vehicle was loaded and transported with the highest standards of professionalism.

Outcome:

The vehicle was safely delivered to Maryoutia, Giza, without any additional damage, exemplifying Egypt Assistance’s capability in managing complex accident scenarios. This operation highlights our rapid response, technical expertise, and unwavering commitment to customer safety and satisfaction.
